These are the forms necessary to enter a project in the Science Fair or apply for our scholarship.
 
ENTRY FORM:
To enter a project in the fair, fill out and submit this form before January 31.

TEAM ENTRY FORM:
Team projects can be entered in the fair by completing and submitting both the regular entry form (above) and this team form which provides additional team information.

PROJECT VERIFICATION FORM:
This form verifies that the project entered is the product of original work done by the entrant, and any assistance provided was not of a significant nature and has been duly noted on the display or in the log book. This form must be submitted with the project on the day of the fair. DO NOT send it in with the entry form(s)!
